We have responded to Healthcare Improvement Scotland’s Consultation on building a national approach to learning from adverse events through reporting and review.  We encourage an approach that all times keeps in focus the desired outcome, which is to ensure the highest possible quality of care at the point of delivery.  This should be the main driver of whether and how an incident review takes place.  

In our response, we say that the critical interface is the relationship between those delivering and those receiving care, and that this includes the family, friends and wider community of those who directly receive care.  We then identify the practical steps that we believe are the most critical ones to help fully realise the potential of a national system and to help support the cultural change to openness, transparency and candour.
