Complaint about Forth Valley NHS Board - complaint was from a man (referred to in this report as Mr C) that Forth Valley NHS Board (the Board) failed to provide him with appropriate diagnosis and treatment following his initial admission to the Stirling Royal Infirmary (SRI) on 4 February 2005.
Complaint about Fife Council - complaint was from a man (referred to in this report as Mr C) about Fife Council’s (the Council) handling of his complaint against a Councillor’s (Councillor 1) perceived interference in a planning application submitted by Mr C.
Complaint about Fife Council - complaint was from a member of the Scottish Parliament (MSP) on behalf of her constituent, Mr C. She complained of the way in which Fife Council (the Council) dealt with Mr C’s requests to buy land to allow him to create a vehicular access and also to increase the size of his garden.
Complaint about Lanarkshire NHS Board - complaint was from a woman (referred to in this report as Mrs C) about the nursing care her late father (Mr C) received at Hairmyres Hospital, East Kilbride (the hospital) in October and November 2004.
Complaint about the Scottish Executive Health Department - complaint was from a group of patients (referred to in this report as the Group) representing the former patients of a general practitioner (GP A). The Group complained that the Scottish Executive Health Department (SEHD) failed to properly manage and advise them on the application process for the replacement of GP A or the appeal against appointment by another GP (GP B).
Complaint about East Ayrshire Council - complaint was from Ms C about East Ayrshire Council (the Council) regarding an offer of housing which was made to her in April 2004. She said that her subsequent complaints about the way the matter was dealt with led to her housing application being penalised.
Complaint about Lanarkshire NHS Board - complaint was from a group of patients (referred to in this report as the Group) representing the former patients of a general practitioner (GP A). The Group complained that Lanarkshire NHS Board (the Board) failed to properly manage, advise and involve them in the retiral of GP A and the application process for his replacement. There were two re-appointment processes leading up to a new appointment. The Group also complained about the quality of GP services they experienced while awaiting a replacement and since October 2004 when patients were transferred to a new GP practice (the Practice).
General Practitioner Forth Valley Area - complaint was from Mrs C, concerning her local GP practice. She visited the practice in 2003 and returned with the same symptoms in 2005. She claims that the initial GP who saw her failed to diagnose her illness and that she was treated insensitively on her second visit.
South Lanarkshire Council - complaint was from the tenant of a multi storey council flat (referred to in this report as Mr C) who complained that the Council had delayed inordinately after September 2004 in effecting a repair to the door entry system at the block of flats. He also complained that they failed to inform him of their complaints procedure.
