Lothian NHS Board - complaint concerned a woman (referred to in this report as Mrs C) who complained against Lothian NHS Board (the Board) regarding the care and treatment received by her late father, Mr C, at the Western General Hospital, Edinburgh (the hospital). Mr C, who had Alzheimer’s disease and was aged 86, was admitted to the hospital on 17 July 2005. In the early hours of the morning on 19 July 2005 he fell out of bed and broke his hip. He later underwent surgery to repair the hip. Mr C died on 2 August 2005.

The City of Edinburgh Council - complaint was from Mr C alleging that The City of Edinburgh Council (the Council) failed to handle properly his representations about his council tax. He said that this placed him in a difficult financial position.
A Dental Practitioner in the Lothian NHS Board Area - complaint was from a woman (referred to in this report as Ms C) who claimed that her dentist (the dentist) had failed to provide her with dental bridgework which was identical to her original teeth as had been promised and agreed. The complaint which I have investigated concerned the dentist’s failure to provide dental bridgework to an acceptable standard.
Lothian NHS Board - complaint was from a man (referred to in this report as Mr C) on behalf of his wife (Mrs C) about one of Lothian NHS Board's (the Board) hospitals in caring for and treating her problems following the delivery of their son in January 2005.
Lothian NHS Board - complaint was from a woman (referred to in this report as Mrs C) about the care and treatment of her daughter (referred to in this report as Miss A) by the Royal Edinburgh Hospital (REH) in the period from November 1998 to March 2003. Miss A was then 19 and her pre-eminent condition was severe anorexia nervosa. Sadly, Miss A’s condition continued to worsen and she died in September 2004 (aged 20). Shortly afterwards Mrs C brought additional complaints to the Ombudsman’s office about Miss A’s ongoing treatment from March 2003 until her death, all of which care was funded and under the overall clinical governance of Lothian NHS Board (the Board).
Complaint about Lothian NHS Board, University Hospitals Division - 1. On 31 July 2003 my office received a complaint from a woman (referred to in this report as Mrs C) against Lothian University Hospitals NHS Trust about the care and treatment her late daughter (Ms C) received at the Western General Hospital (WGH) in Edinburgh. Mrs C was also dissatisfied at the outcome of an Independent Review Panel (IRP) into her original complaint.

Complaint about a Dental Practitioner in the Lothian NHS Board Area - complaint was from a man (referred to in this report as Mr C) who complained that his Dentist (referred to in this report as the Dentist) failed to provide him with a denture that was fit for purpose.
