A Dental Practitioner in the Greater Glasgow and Clyde NHS Board Area - complaint was from an advocacy worker on behalf of her client (referred to in this report as Mrs C). Mrs C complained that her former dentist (Dentist 1) had improperly administered her with an anaesthetic causing her to suffer an adverse reaction and long-term illness.

North East Scotland
Greater Glasgow and Clyde NHS Board - complaint was from a woman (referred to in this report as Mrs C) against Greater Glasgow and Clyde NHS Board (the Board). Mrs C is confined to a wheelchair with Multiple Sclerosis (MS). She complained that the Board’s policy on provision of physiotherapy for people with long-term disabilities was discriminatory.
Grampian NHS Board - complaint was from a man (Mr C) against Grampian NHS Board (the Board) regarding the care and treatment provided for his mother, Mrs C, at Woodend Hospital, Aberdeen (the hospital) in March 2004. Mrs C was admitted to the hospital on 25 March 2004 at about 01:00. Sadly she died there at about 10:55 that day.
A GP in the Grampian NHS Board Area - complaint was from a woman (Mrs C) who claimed that her late husband (Mr C)'s general practitioner (GP) was responsible for his death because of his incorrect diagnosis and inadequate standard of care at several telephone and face-to-face consultations between December 2003 and February 2004.
The Student Awards Agency for Scotland (SAAS) - complaint was from a man (Mr C) who claimed that the Student Awards Agency for Scotland (SAAS) were demanding a £2,000 Graduate Endowment payment from him and threatening legal action if he failed to pay. Mr C was aggrieved because, despite providing SAAS with information which he considered showed that he was clearly not liable, they continued to pursue him for payment.
Glasgow City Council - The complaint concerned the Council's handling of an application for planning permission for the part-use of a shop for the manufacture of Asian sweets and the erection of a ventilation duct at the rear of the premises. Ms C's property was adjacent to the shop. She contended that the Council, as the local planning authority, wrongly granted consent for this use and subsequently failed to ensure that the ventilation duct was positioned and operated properly.
Tayside NHS Board - complaint was from a man (referred to in this report as Mr C) about the death of his wife, Mrs C, in a hospital of Tayside NHS Board's (the Board) Acute Services Division (the Division) in March 2005.
University of Glasgow - complaint was from a woman (referred to in this report as Ms C) – a research assistant formerly employed by the University of Glasgow (the University) at which she was studying for a post graduate degree. Ms C made a formal complaint against the University about the quality of the supervision she received during her PhD studies and exhausted the authority’s internal complaints procedure. The complaint was dismissed formally by the University on 5 April 2005. Ms C subsequently made a request for an external review to the Ombudsman, alleging that there was administrative fault or service failure by the University which caused her injustice and hardship
